The big picture
Kratom has been sold in the U.S. for decades, primarily as dried leaf powders and capsules used for pain, anxiety, and opioid withdrawal. Since approximately 2024, a new category of concentrated 7-OH products — often marketed as 'kratom' — has rapidly entered the market as gummies, sublingual tablets, nasal sprays, and shots. These semi-synthetic formulations, produced by chemically oxidizing mitragynine isolates, deliver far higher 7-OH concentrations than natural kratom and bypass first-pass metabolism when used sublingually or intranasally. Regulatory agencies including the FDA, and state health departments in Texas, Pennsylvania, and Los Angeles County, have flagged increasing poisonings, hospitalizations, and fatalities linked to these concentrated products.
