The big picture
Most prior research linking kratom to deaths involved polysubstance cases where fentanyl or other opioids were present. This new five-year Florida study separates "mitragynine-induced fatalities" (no other toxic substance present) from "mitragynine-associated deaths" (co-exposures present). Researchers found that pure mitragynine deaths are characterized by very high blood concentrations and specific autopsy findings, while the majority of kratom-involved deaths result from drug combinations, particularly fentanyl.
