Kratom and its synthetic derivative 7-hydroxymitragynine (7-OH) do not appear on standard opioid toxicology panels, creating a clinical blind spot in emergency, addiction, and primary care settings. Standard kratom immunoassay tests target mitragynine, the primary natural alkaloid, raising questions about whether newer 7-OH-only products might evade detection. This has created significant uncertainty for providers managing patients who may be using these substances without disclosure.

According to Labcorp's clinical guidance: "Although the immunoassay is not designed to detect 7-hydroxymitragynine (7-OH) directly, our internal evaluation of commercially available 7-OH products and published reports show that these products commonly contain mitragynine. As a result, the presence of mitragynine is typically sufficient to produce a positive screen in most cases."
