His publicly presented observations cut both ways — not defending kratom as harmless, but challenging the simplistic 'high mitragynine = massive leaf overdose = ban the leaf' inference.
Key findings
- On gastric contents: 'They're not finding huge amounts of kratom material or capsules in gastric contents — it's usually pretty empty inside.' No green sludge, ghost pills, or botanical loads — challenging death narratives that depend on massive natural-leaf ingestion.
- On autopsy presentation: kratom-associated cases often do not show classic opioid overdose signs (foam, heavy lungs, pulmonary edema, urinary retention) — they appear more like sudden collapse events.
- On postmortem redistribution: mitragynine has a long half-life and large volume of distribution; it accumulates with chronic use and may redistribute after death, inflating blood concentrations.
